Eating 2 Brazil nuts daily contains a total 136-182 mcg selenium.2 Eating a 3 oz serving of yellowfin tuna contains 92 mcg  of selenium which can help with reaching daily nutrient goals. Top sirloin beef contains 28 mcg of selenium per 3 oz and is another great source of this mineral.3

 Vitamin D is essential for immune system modulation and overall health maintenance. Deficiency of vitamin D is common among Hashimoto's patients and is often linked to autoimmune disorders. A study demonstrated that vitamin D supplementation effectively reduced thyroid antibody levels and improved thyroid function in individuals with autoimmune thyroiditis.4 The RDA for vitamin D is between 15-20 mcg per day for adults. Great sources of vitamin D are found in fatty fish including cod liver oil, salmon, and swordfish. Incorporating these foods can increase vitamin D levels and 1 tablespoon of cod liver oil contains 34 mcg,  a 3 oz serving of swordfish contains 14 mcg, and a 3 oz serving of salmon contain 11 mcg of vitamin D.3

 Omega-3 fatty acids, which are commonly found in fish oil supplements, possess anti-inflammatory properties that can help manage the inflammation associated with Hashimoto's thyroiditis. Additionally, these fatty acids support a healthy immune response. A study published in the Journal of Nutritional Biochemistry found that omega-3 supplementation led to a reduction in thyroid inflammation and oxidative stress in autoimmune thyroiditis.5 Adequate intake of omega-3 fatty acids is 1.7 g per day. Herring and salmon are amazing sources of omega-3 fatty acids since a 3 oz serving contains at least 1 g of omega-3 fatty acids.

Ashwagandha, an adaptogenic herb, offers potential benefits for stress reduction and immune modulation. Some studies suggest that Ashwagandha supplementation may help stabilize cortisol levels and mitigate stress-related impacts on thyroid function. A pilot study conducted by Sharma et al. in 2018 reported improvements in thyroid function and quality of life among participants with subclinical hypothyroidism who took Ashwagandha.6 Doses of 300 mg 2 times per day have been shown to decrease stress levels. 7

Zinc is a crucial mineral involved in gene expression, cell division and growth, and a variety of enzymes related to immune and reproductive functioning. Studies have explored the connection between zinc concentrations and thyroid hormone levels. Food sources of zinc include oysters which contain 38.4 mg per 1 oz, ground beef with 5.3 mg per 3 oz, quinoa with 1 mg per ½ cup, and pumpkin seeds which contain 6.7 mg per cup.8

What happens if I don't treat my hyperthyroidism?

Because your thyroid plays a major role in your metabolism, if it's not regulated or stable, it can wreak havoc on many of your other body systems. Some complications may include:

  1. Cardiac problems:
    1. Being in a hyperthyroid state can make your heart work faster and harder, causing palpitations and increased heart rate. This is why its so important to follow your physician's instructions in frequent lab testing and follow-up. While these complications can be seen in hyperthyroid patients, it can als  occur in hypothyroid patients who are on too much thyroid medication.

    1. This is a potentially life-threatening condition when your body releases too much thyroid hormone putting you into a hypermetabolic state. Some symptoms may include sudden fever, deliria, rapid pulse and sweating. This is a medical emergency and you must seek immediate care.

    1.  An excess amount of thyroid hormone can impair your body's ability to absorb calcium. Your bones become weak when they don't obtain enough calcium and can put you at great risk for fractures and osteoporosis.

Can hyperthyroidism cause hypothyroidism?

Yes. If you're on too much anti-thyroid medication or have had part of your thyroid removed, its likely that you will become hypothyroid.

Hypothyroid symptoms may include weight gain, hair loss, constipation, fatigue, joint pain, cold intolerance. Frequent follow-ups and lab testing with your Physician can help you keep your thyroid better managed. 

Case Study:

A 57 yo male presented to a clinic with a diagnosis of Graves’ Disease. His prior Physician recommended radioactive iodine, however, the patient wanted a second opinion. He was having significant symptoms including heart palpitations, significant weight loss, extreme hunger, increased heart rate and visual changes. He was undergoing a lot of stress from work and had previous infections. Through shared decision making, we decided to start the patient on methimazole (anti-thyroid medication), a beta blocker, immune support (specifically for autoimmune thyroiditis) and followed up three weeks later. The patient was also referred for a cardiac and ophthalmologic work-up.

I called the patient the next day to see how he was feeling on the medicine, and he said he already felt better and his heart rate was lowering. At his follow-up appointment three weeks later, the patient stated he was feeling significantly better and was no longer getting the adrenaline rush with obeshe was feeling a few weeks prior. The patient was responding well to the medicine and he was instructed to continue at the same dose. The Patient's nutrient status was also checked and he was put on several vitamins and liver support.

The patient came in several weeks later and said overall he was feeling 7.5/10 (10 being the best) and previously was feeling a 3/10. The patient's labs were run and he was responding very well to the medicine and we were able to reduce his thyroid medicine.

The patient is scheduled for a follow-up this month and will repeat thyroid labs and imaging so we can visualize the thyroid gland.

What is Andropause?

This can also be known as Male Menopause. After the age of 40, hormones such as testosterone start to decline about 10% with each decade of life.

This can lead to symptoms of fatigue, depression, weight gain, and low libido. This can lead to a diagnosis of male hypogonadism, a condition in which the male primary reproductive organs are not functioning optimally.

What is a gland?

Where is your thyroid located?

Can I feel my thyroid?

Yes, you can do your thyroid neck check at home, however, this should not replace a visit with your healthcare provider, who can better assess your thyroid through physical examination, bloodwork, and imaging. It should be noted that it is normal not to feel your thyroid.

Now, to palpate your thyroid it is best to stand in front of the mirror with your neck extending and chin pointing up towards the ceiling. With your neck extended back, take a sip of water and visualize the shape of your thyroid as you swallow. By doing so, you will be better able to see any irregularities in your thyroid.

You can assess for any enlargements, swelling, bumps, or protrusions as you swallow. Next, you can begin to gently touch the area around your thyroid, checking for any swelling, lumps, enlargements, or irregularities. You can repeat this step a few times and observe your neck.

The thyroid is located directly below Adam's apple so be sure not to confuse your Adam's apple with your thyroid. If you see any bulges or irregularities, consult with your healthcare provider.

How do you test for hypothyroidism?

Blood tests are often used to diagnose hypothyroidism. Your healthcare provider will likely assess your thyroid by checking TSH, also known as Thyroid Stimulating Hormone.

TSH is a hormone released by your pituitary gland that signals your thyroid to make and release hormones into your bloodstream. When your bloodstream has an insufficient amount of thyroid hormone, also known as hypothyroidism, your thyroid will signal to your pituitary gland.

This signal will cause your TSH levels to increase the signals to your thyroid to make more thyroid hormones (T4 and T3). So therefore, your healthcare provider will most likely assess your TSH to assess your thyroid function but may also evaluate your thyroid hormones, including free T4 and free T3.

TSH levels range between 0.45 and 4.5 mu/L in a normal functioning thyroid. However, every healthcare provider has a different value that they believe is optimal.

At Natural Endocrinology Specialists, we like to see a TSH value between 1-2 mu/L. A TSH value above 4.5 often indicates low free thyroid hormone, indicating hypothyroidism.

While healthcare providers will also look at your free thyroid hormones, TSH is still clinically the best indicator to assess for hypothyroidism. 

Is hypothyroidism familial?

Yes, often, hypothyroidism runs in families. If you have a family history of thyroid disease, you should let your healthcare provider know so you can get tested.

What is Hashimotos?

Hashimotos Thyroiditis is an autoimmune condition in which your immune system attacks your thyroid gland. Hashimoto's disease most often causes hypothyroidism or an underactive thyroid. 

What causes Hashimotos?

The exact etiology of Hashimotos is unknown. However, it is believed that there are several contributing factors. Some of these factors may include your genetics.

A family history of thyroid disease could contribute to Hashimoto's or hypothyroidism in many cases. Other contributing factors include hormones, excessive iodine, and radiation exposure. 

Does Hashimotos affect males and females equally?

Hashimotos is known to affect women 7x as likely as it does males. It's not uncommon for women to be diagnosed with a thyroid condition during the first year after having, with 20% of these women developing Hashimoto's years later. 

Do the symptoms between hypothyroidism and Hashimotos differ?

Frequently, the symptoms of hypothyroidism are similar to Hashimotos. However, some patients with Hashimoto's may develop an enlarged thyroid, also known as a goiter.

The goiter may look like a significant swelling at the front of your neck and may cause other symptoms like hoarseness and difficulty swallowing.

How do you test for Hashimotos? 

To test for Hashimotos, your healthcare provider will often run a thyroid panel to assess if your thyroid is under or overactive.

Your healthcare provider will also run other blood tests, specifically thyroglobulin antibodies, to assess general autoimmune thyroid disease and Thyroid peroxidase antibodies (TPO), which tests specifically for Hashimotos Disease.

If your healthcare provider suspects thyroid disease, they should also send you a thyroid ultrasound to assess for vascularity of any thyroid nodules.

How are hypothyroidism and Hashimotos treated?

Hypothyroidism and Hashimoto's are often treated with thyroid replacement medication. There are several different types of medication, so it is always best to work with your healthcare provider to assess what type of medication is best for you.

You mentioned iodine... how much iodine should I be having per day?

The recommended daily allowance for iodine intake is 150 mcg in adults and slightly more in pregnant and breastfeeding women. Research shows that too much iodine intake may contribute to hypothyroidism.

Where can I be getting excess iodine from?

Fish, seafood, and seaweed contain high levels of iodine. Other dietary sources of iodine include eggs, dairy products, and iodized salt. Other sources of iodine could be found in your daily multivitamins and may even be found in some of your favorite skincare products.

So, What Are Thyroid Nodules?

Simply explained, Thyroid nodules are abnormal lumps of thyroid tissue. Your thyroid has a normal buildup of cells, called follicles. Follicles functions are:

Who Can Get Thyroid Nodules?

The risk of people getting thyroid nodules runs parallel to their age. For women, their age in decades corresponds clearly with their risk for developing nodules.

At age 30, there is a 30% risk of developing thyroid nodules. And 50 years old people have a 50% chance of developing thyroid nodules. The same goes for being 60 (60%) and 70 (70%). Quite simple to remember but essential not to forget.

Overall, for women, it is three times more common to develop thyroid nodules compared to men. Especially for women, getting to know more about their thyroid status is essential. But still, men should know the chances of developing thyroid problems and how it varies between genders.

Are Your Thyroid Nodules Dangerous?

You may be concerned that thyroid nodules will be especially dangerous for your health.

Thyroid nodules are usually not dangerous. There are natural methods for thyroid nodules.

Often, 95% or more pose no risk (benign). If you are concerned about thyroid nodules and having them, first, you need to determine whether or not you for sure have them.

Also, just having them doesn't mean that they are necessarily going to cause you harm. The majority of nodules are harmless.

What Are Symptoms Of Thyroid Nodules?

It is common to want to know possible symptoms for those at risk of developing thyroid nodules. People want to know whether they might feel something due to the thyroid nodules being present on their thyroid. What you should know is there are usually no symptoms.

If the nodules are big enough, they can affect speaking or swallowing. If they are massive, they can affect the nerves that allow your tongue to move properly. This is rare, and they are usually not ever painful.

If they are big or not in the right place, nodules can cause some discomfort on a certain level. However, this is pretty unusual. It's most likely not something you need to be actively worrying about.

How To Find Out If You Have Thyroid Nodules?

There are three effective methods to consider when finding out if you are dealing with thyroid nodules: doctor's exam, self-exam, and ultrasound.

Doctor's exams are able to pick up nodules somewhere around 10% of the time. Self-exams are similar unless you are doing it regularly and can pick up any irregularities yourself. The gold standard is, ultimately, ultrasound.

It is encouraged that all of those with thyroid disease have a screening ultrasound to determine the state of their thyroid.

This is the only method to definitively determine whether you have a potentially dangerous nodule that you have never noticed before.

What Steps Can Be Taken To Treat Thyroid Nodules?

There are a few things you can consider should the need arise.

Possibly needing to have a biopsy can be determined on multiple factors, such as your nodule is well over 1 centimeter in size, your nodule has calcified, and if your nodule has exaggerated blood vessels.

Now, considering all of the factors, you might want to have your nodule biopsied. This is because the nodule, at this point, looks like it could be developing into cancer.

Typically you should have some combination of these three to require surgery of any sort.

If you have a nodule that's biopsied, the next thing to consider is removal. How to know when to remove it? 

These factors are important in deciding whether the thyroid nodule or portions need to be removed.

How To Reduce Nodules Naturally?

The most significant factors in handling thyroid nodules are: dialing in your iodine^2, reverse insulin resistance, and dialing in your TSH.

When it comes to TSH, let's think about the normal/optimal range. We have an extensive normal range and a very slim optimal range.

When it comes to nodules, what we want to do is get even narrower (between 0.5 and 0.9 but not lower than that).

How Does TSH Affect Your Nodules?

TSH stimulates the thyroid and while that might be good for a perfectly healthy thyroid, it can also promote nodules.

TSH and your nodules don't get along in the sense that more TSH causes your nodules to grow and could make them problematic. The lower you get your TSH to a normal level, the better opportunity you have of suppressing any potential nodule growth.

What Do We Know About Iodine?

When your iodine is well dialed in, you start to decrease your risk for thyroid nodules. Iodine is essential, and it is even more important to know the amount you are getting in your diet already.

Low iodine intake could lead to an increased potential for thyroid nodules. However you shouldn't get too much, but you also don't want to have too little.

Having too little Iodine is not right, and so is too much, and even a responsible change can be dangerous for specific segments of the population. That balance is delicate and it is so crucial that you get it correct.

Also, reversing our insulin resistance is ideal. There is data suggesting that you are more apt to grow nodules on your thyroid when your body has a more challenging time dealing with blood sugar.

When we are able to deal with how our bodies deal with insulin, it can go help in managing our risk for developing nodules.

What About Produce?

Significant amounts of produce in the diet can reduce the risk of nodules. It suppresses the growth of nodules and ultimately helps reverse them altogether.

You should be aware of the benefit of more produce in your diet. It's just one extra reason to add a bit more fresh fruit into your diet.

Have your produce, and don't worry about it. You are better off having more types of different fruits and vegetables than fewer.

When you get more into your diet, you ward off nodules that may start appearing naturally. It's a tasty way of keeping your thyroid in check while being helpful.

It's an exploration into how Italy's olive oil business has been corrupted by the Italian Mafia, making a fortune by meddling with Italian food products in ways that can affect consumers worldwide.

As the 60 Minutes producer, Guy Campanile, says: 'The Italians call it Agromafi, and it is estimated to be a $16-billion-per-year enterprise. One result: "Much of the olive oil we import isn't as pure as it seems.

The most common type of olive oil fraud is mixing it with lower-cost oils, often from North Africa and Mediterranean.

In some cases, what is labeled extra-virgin olive oil may not be olive oil at all instead, seed oil like sunflower made to look and smell like olive oil with a few drops of chlorophyll and beta-carotene.

Some may be shocked to hear that Italy only produces 15% of the world's olive oil.

In recent years, factors such as attacks from the Xylella fastidiosa bacteria and weather fluctuations have meant that Italy's olive oil production has plummeted to record lows.

Even Europe isn't left out a 2013 European Parliament report concluded olive oil as Europe's most adulterated, tampered with, and counterfeited agricultural product.

Despite attempted regulation and legislation changes, a 2020 report by the U.S.'s American Botanical Council discovered that adulteration of olive oil is still rampant.

It also highlights the two main reasons for this fraud: low availability of high-quality olive oil and soaring demand.

The Dangers of Olive Oil Fraud

The dangers might be more significant than you think, especially if you have a peanut or soybean allergy since those are often the oils mixed into the actual virgin olive oil. But the threat doesn't stop with allergies.

In July 2010, researchers at the UC Davis Olive Center set out to inspect samples of imported olive oils, only to discover that 69% failed to meet international standards for the oil to be considered extra virgin.

Both NBC News and the Wall Street Journal have reported that cheap, fake extra virgin olive oil is rampant and American stores are flooded with it.

Even Forbes magazine urged: the olive oil in your pantry, the one you bought for its health benefits and some sliver of the seductive Mediterranean lifestyle, is most likely a scam.

And what's most annoying about this? It's preventing you from the insane health benefits of authentic extra virgin olive oil!

But if it's authentic Italian olive oil you want, prepare your wallet: If you're paying seven bucks or eight bucks for a bottle of Italian extra virgin olive oil, he says, it's probably not Italian extra-virgin.

And when you taste the real olive oil, you'll fall in love. True extra-virgin olive oil is made exclusively from the first pressing of the olive harvest and is devoid of additives.

You know, when you see it there [in Italy], it's this almost luminescent green," Campanile says, "it looks like nothing you've seen before and tastes like nothing you've tasted before.

When you walk into the mill, you are just hit with this extraordinary aroma of olive oil that is just so pleasing, it makes the hair on the back of your neck kind of stand up," he tells Overtime editor Ann Silvio, I don't know what it is, but it's almost primeval how wonderful it is.

Case Study 🔍

A 43 year old female presented to the clinic with a diagnosis of endometriosis and full hysterectomy that put her through early-onset menopause. She was put on hormone replacement therapy by her previous physician and her testosterone levels were extremely elevated in the 800s and elevated estrogen levels. She was experiencing acne, hirsutism, and voice changes. She also said that she was still gaining weight despite efforts to lose weight, elevated hematocrit levels and edema. She also had a history of hypothyroidism and significant stress.

We had her stop her testosterone and estrogen and noticed she lost 5 lb as soon as she discontinued testosterone therapy and a total of 10 lbs within 3.5 weeks. Once we were able to get her testosterone and estrogen levels back down, we initiated much smaller doses of hormone replacement therapy via injections. Despite normal hormone levels, the patient noticed that she would still experience “puffiness” several days after hormone injections. We decided to send her for a cardiovascular work-up to assess the patient's edema. The cardiologist noted an abnormal EKG for sent the patient to the ER for evaluation.

Patient will continue will be monitored by the Cardiologist and adjunctive care will allow the patient to feel optimal but also prevent any cardiovascular disease issues in the future.

At NES, we offer a 4-point salivary cortisol test to help us better understand the secretion of this hormone throughout the day and to get a deeper understanding of your circadian rhythm.

The downside of doing this in a simple blood test is that you can only get one reading and that may look normal.

A full day picture is a better analysis of how cortisol is being released in your body by the adrenal glands. There are different types of rhythms and the most optimal one is when you are thriving and in the zone based on the circadian slope.

You can have other conditions where your cortisol may be below in the morning and high in the evenings, or you can either run high cortisol all day or low cortisol all day.

Once this is better understood, a specific treatment plan will be recommended for you to get your adrenal health in excellent shape and back on the nice curve.

What to expect from us

At your doctor's appointment, a take-home salivary cortisol test may be recommended for you to better understand and evaluate your adrenal health.

Your adrenal gland release cortisol and do so in a circadian pattern, high in the mornings, low at night. It mimics the light and dark cycle; it should be the highest during the hours of sunrise and lowest during the hours of sunset.

This test is a salivary cortisol test that requires you to collect 4 saliva samples at specific times during the day. The kit comes with prepaid mailing instructions and is to be completed and shipped out by the patient upon completion. The results will be sent directly to your doctor.

At your next scheduled follow-up appointment, the results will be reviewed and if a treatment should be required, it will usually be discussed at this visit. 

A repeat test will typically be required so that a pre and post-analysis of your cortisol levels can be determined to see if therapy and results are improving symptoms.

Case Study

A 47 year old male presented to the clinic with obesity and type 2 diabetes. He weighed 256 lb on his first visit. When he first presented to the clinic, he said he’s been overweight since childhood and always had trouble losing weight. He also had a family history of diabetes, craving for sweets and a poor relationship with food.

He also had a stressful job that made it difficult to eat and sleep during normal hours. At the first visit, the patient was put on a continuing glucose monitor, liver detox program and berberine and ALA supplementation. When we first checked his Hemoglobin A1c, it was a 10.1%. The patient followed up two weeks later, and he reported feeling better and a 5 lb weight loss.

Two months later, his Hemoglobin A1c went down to 7.5% and his weight was down 10 lb. Within less than a year, his Hemoglobin A1c went down to a 5.9%, putting him in the pre-diabetic range and his weight went down from 256 lb to 238.5 lb. Patient was also tested for sleep apnea, as interrupted sleep can have a significant impact on blood sugar. Patient was put on a CPAP machine and noticed a difference in how he felt on the CPAP machine.

We take an Integrative Approach

A 41 year old female presented to clinic with symptoms of:

She was diagnosed with Hashimotos Thyroiditis and said she felt horrible and progressively worse.

We had her lower her thyroid medicine and re-test again in 4 weeks. We also discovered that her iron and ferritin were both extremely low, and initiated her on iron IVs.

After a few months, we decided to change her thyroid medicine to a cleaner form of medicine and repeated her labs again 6 weeks later.

After only 6 weeks, she said she wished she had been on this thyroid medicine 10 years ago. She said she was feeling amazing. She was also put on immune and inflammation support for her thyroid and oral progesterone therapy to help during her menstrual cycle.
